Working as part of the Ynot Aspire team, the role includes visiting young people who have been referred to the project through various organisations. On a one-to-one basis, assessing and supporting them to improve health and wellbeing needs of people with non-medical intervention such as linking them to activities and opportunities. Also to liaise with lead on the Hyndburn Way Young People’s Mental Health Project.
The role will include:
- Providing an assessment and outreach service, usually in schools, gaining trust and commitment from young people.
- Assessing the client’s abilities and preferences, thinking laterally and then advising and supporting them in a variety of areas/activities.
- Maintaining a detailed knowledge of the local community and voluntary sector services/activities, representing HRV CVS and building a network of positive relationships with partners.
- Developing strong relationships and linkages between GP practice staff and the wider VCF sector.
- Maintaining an awareness of networks to improve an individual’s health and wellbeing.
- Working with our CVS and the project lead for the Hyndburn Way Young People’s Mental Health project to map local resources that support young people’s mental health and wellbeing.
The role is for 18.75 (or less to suit) hours per week for 12 months but with possibility of being extended subject to funding. Salary £14,203.56.
